I certify that the applicant has my permission to attend Mike Henshaw’s Soccer Camps 2010. I hereby waive and release the “Soccer Camp 2010″ and it’s employees from liability or injury. I know of no mental or physical problems which affect my child’s ability to safely participate in this camp. I further certify that the above named participant has medical insurance in case of injury. I hereby authorize the directors of Summer “Soccer Camp 2010″ to act for me accordingly to their best judgement in any emergency requiring medical attention. New camp certificate will be issued in the event of full camp rain out.
